Investing in Bone Health for a Lifetime with Bones for Life

Bone health is a lifelong investment. It doesn’t begin after an osteoporosis diagnosis or a concerning scan—it starts the moment we begin moving in gravity and continues to influence how we stand, walk, and age.

 

Many people don’t think about their bones until they notice changes in posture, balance, height, or confidence in movement. Yet our bones are living tissues that continually respond to the way we move every day.

 

In a conversation for the Move Better, Feel Better Summit, Lavinia Plonka and I explored the connection between bone health, posture, balance, osteoporosis prevention, and the work of Ruthy Alon, creator of the Bones for Life® program. The insights from that discussion have been gathered here to help you better understand how movement supports stronger bones and healthier aging.

Why Bone Health Matters at Every Stage of Life

When people ask me when they should start investing in their bones, my answer is simple:

” If today is the first day you’ve thought about investing in your bones, then today is the day you should start doing it. “

I mean that in the most practical way possible. 

 

We need bones because we live in gravity. Bones give us structure and leverage against gravity, while muscles help us move that structure. Without both, getting up and down, walking, and living upright would not happen the way it does.

 

The body has a natural timeline for building and maintaining bone. From birth into puberty, and then through the 20s, the system is set up to lay down bone. After that, the process gradually levels off before eventually declining.

Bone Health Across the Lifespan

For women, there is often a sharper drop in the five to six years after menopause, about 6 percent. After that, women and men tend to lose about 1 percent a year. That doesn’t mean you are powerless. It means the earlier you start, the better. And if you didn’t start earlier, you begin now.

The Critical Role of Gravity and Movement in Bone Health

One of the clearest examples of how much gravity matters is in development. Children who cannot come into weight-bearing early enough may not fully form hip sockets the way they need to. That’s how strong the relationship is between biology and gravity.

 

The same principle shows up in adolescence. In studies with girls and boys, even a few jumps a day increased bone density. That matters because it reminds us that bones respond to use.

 

Diet matters. Sun exposure can matter. Medications matter. Underlying health conditions matter. But if someone stays in bed and eats perfectly, they will still lose bone. If that same person regularly challenges their skeleton through weight-bearing movement, the picture changes dramatically.  

 

That is one of the most important lessons in bone health. Movement isn’t just exercise. Movement is information. Every step, every shift in balance, and every interaction with gravity sends messages to the skeleton about how strong and adaptable it needs to be.

Understanding Osteoporosis and Fracture Risk

What Causes Most Bone Fractures?

A lot of people come to bone health after the fact. They get a scan. They hear the word osteoporosis. They start looking for exercise classes for osteoporosis, posture classes, weight training, or anything that sounds like a solution. 

 

I understand that impulse. The trouble is, the research behind many standard recommendations is thinner than people think, especially for older adults who already have osteoporosis. Much of the research has been done on younger women, not women in their 60s, 70s, or 80s who are already living with bone loss.

 

That doesn’t mean exercise is pointless. It means we should be honest about what we know and what we don’t.

 

A traditional program I respect is the Too Fit to Fracture initiative that came out of Canada. It takes a careful physical therapy approach, and I appreciate that honesty. Their basic point is one I agree with: aging changes bone, and fracture risk is not only about density.

 

The main thing people need to hear is this: most fractures are caused by falls, not by osteoporosis alone. This makes fall prevention for older adults one of the most direct and actionable investments in long-term bone health.

That changes the whole conversation. If you only focus on bone mineral density, you miss a huge part of the picture. If you improve balance, posture, walking, and the body’s ability to recover when something unexpected happens, you are already investing in your bones.

 

Lavinia raised a smart point in our conversation. When the head is forward and the body is not well organized, you’re not only putting odd forces through the skeleton. You’re also less ready to respond when you trip, misstep, or don’t see the curb in time. That’s why fall prevention is not separate from bone health. It is bone health.

 

So what does that mean for you? Don’t ask only, “How do I get stronger?” Ask, “How do I move through gravity with better organization?” Those are not the same question.

Osteoporosis vs. Osteoarthritis: What's the Difference?

People often confuse osteoporosis and osteoarthritis. They are not the same condition, even though they can occur in the same person and both affect quality of life. 

How Osteoporosis Affects Bone Structure

Osteoporosis affects the inner trabecular, or spongy, structure of bone. This is often what a DEXA scan is measuring.

 

Inside the bone, the trabecular structure resembles a living lattice. When that structure becomes thinner, it can no longer handle the same load as effectively.

 

In the spine, this is especially important at the front and middle portions of the vertebrae. People often think of “breaking the back” as something that happens on the back side of the spine. In osteoporosis, compression fractures commonly occur across the front of the vertebrae.

How Osteoarthritis Affects Joints

Osteoarthritis affects joints, cartilage, and the harder cortical bone around those joint surfaces.

 

Unlike osteoporosis, which is often discovered later through testing, osteoarthritis frequently becomes noticeable years earlier because of pain, stiffness, or changes in movement.

What Both Conditions Have in Common

Although osteoporosis and osteoarthritis are different conditions, I believe they often share a common mechanical challenge: poor force transmission.

 

When movement is organized well, force travels through the body more efficiently. When it is not, the body experiences more shear force.

 

Over time, excessive shear can contribute to irritation, compression, wear, and breakdown.

 

That’s the important takeaway.

 

The way you stand and walk is not simply a cosmetic issue. It changes the messages moving through your joints and bones all day long.

How Bone Health Is Measured

Most people hear about osteoporosis through bone mineral density, usually from a DEXA scan. That is the standard tool, but it only tells part of the story.

 

A DEXA scan measures density. It does not directly tell you about tensile strength, compressive strength, or every other quality that matters in a living bone. It is useful, but it is not the whole picture.

 

Timing matters as well. Many women are not tested until menopause, and many men are not tested until much later unless concerns arise because of height loss, family history, or other risk factors. 

 

If someone wants a traditional medical approach, I often recommend speaking with an endocrinologist. Internal medicine physicians and general practitioners are usually the first stop, but endocrinologists can take a closer look at osteoblast activity—the cells that build bone—and osteoclast activity—the cells that break bone down. 

 

That can sometimes reveal whether a treatment strategy is affecting the process before changes appear on a DEXA scan.

 

A DEXA scan may not show meaningful improvement for two to five years, and the margin of error can be significant. A small increase or decrease may not tell the whole story.

How Posture and Balance Affect Bone Health

Why Dynamic Posture Matters

Let’s talk about posture, because people hear that word and immediately stiffen.

 

Someone tells them, “Take your chin back.” Or, “Pull your shoulders down.” Or, “Stand up straight.” That may be better than nothing in some situations, but it is not the same as organized posture. 

 

Moshe Feldenkrais referred to that forced version as a “post,” That description says a lot. 

 

What I care about is dynamic posture. The Feldenkrais Method, and specifically Bones for Life Feldenkrais work, offers a different approach. It focuses on organization rather than rigidity.

 

The same principle applies to balance.

 

That’s why I often use the phrase dynamic postural balance. Balance is not about freezing in place or standing on one leg. Posture is not about standing straight. It is about your ability to move, adapt, and recover.

 

True posture and balance are living processes. The relationship between balance and bone health is inseparable — improving one directly supports the other.

Why the Head Matters

The head weighs about 10 pounds. When that weight sits well on the spine, it sends a clearer message through the system: “I expect strength here.”

 

When the head moves forward and the chest collapses, the message changes. The thoracic spine, particularly the front of the rib cage and vertebrae, no longer receives the same information about support and strength.

 

That affects bone health. It affects posture. And it affects balance.

 

A head carried forward is not only harder on the neck. It changes the whole organization underneath it.

 

One of the things Ruthy Alon saw so beautifully in the Bones for Life program was that the weight of the head can become an ally when it is positioned well. As one trains further in her work, one learns to carrying small loads on the head. Ruthy Alon Bones for Life work was inspired in part by the African women who carried water with remarkable efficiency and grace.

Researchers described those women as being able to carry a load “for free,” up to 20 percent of their body weight, without extra oxygen use. Ruthy was fascinated by the beauty of that movement and what it revealed about alignment. 

 

One of the most valuable lessons in Bones for Life aligning the neck is learning how to organize the head without forcing the chin backward.

Why Forcing Good Posture Doesn't Work

Good posture is not rigid, and balance is not frozen. If you cannot go forward and back, side to side, and recover, you are not balanced. You are stuck.

 

That is why I always talk about dynamic postural balance. The body needs mobility and posture working together. Not one instead of the other.

 

When force travels through the body in a collapsed or inefficient way, more shear is created. Over time, that may contribute to pain, inflammation, cartilage wear, and the issues we later label as osteoarthritis or osteoporosis. 

 

So the goal is not to become perfect. It is to make the message cleaner. Cleaner force. Cleaner support. Cleaner organization. That is enough to change a lot.

What Is the Bones for Life Program?

Beyond Traditional Exercise for Bone Health

When people search for “Bones for Life Ruthy Alon” or “Ruthy Alon Bones for Life,” they are often looking for exercises.

 

And we do offer somatic exercises in that program. But what Ruthy Alon created is much bigger than an exercise program.

 

The Bones for Life program grew out of the work of Moshe Feldenkrais such as the Feldenkrais Method and focuses on changing the movement habits that shape posture, walking, balance, and weight-bearing throughout daily life.

 

Traditional exercise programs often focus on muscle strengthening and assume that stronger muscles will automatically lead to stronger bones. Sometimes that may help.

 

Bones for Life asks a different question: how can we improve the quality of weight-bearing itself?

 

That is why I keep returning to the relationship between posture and mobility. Bones for Life bone health work is not about making you work harder. It is about helping force travel through your body with less shear, better organization, and greater support. It is about adaptability. It is about changing the WAY we move. It is a somatic education approach that engages the mind and senses.

Why Bones for Life Feels Different

I’ve taught Bones for Life for years, and one thing I value about it is accessibility. Many people are hesitant to participate in exercise classes for osteoporosis because they have been injured before, are dealing with pain, or fear making their condition worse. 

 

Bones for Life offers another way in. The approach is gentle, progressive, and accessible to a wide range of ages and abilities. It helps people improve balance, walking, posture, and confidence without relying on high-impact movement or forceful exercise. 

 

One quiet strength of the method is aligning the neck without forcing the chin back. Another is helping the low back, ribs, pelvis, feet, and head begin to relate differently. Over time, that changes how people walk.

 

And walking matters.

Why Walking Is Powerful Medicine

“Quality walking is quality medicine.”

I mean that literally. Every step is an opportunity to send a better message through the skeleton.

 

Every step becomes an opportunity to improve support, organization, and adaptability.

 

The idea of quality walking comes not only from Bones for Life training but also from the work Carol Montgomery and I have done in Integral Human Gait theory.

 

When walking becomes more organized, weight-bearing becomes more effective.

 

And when weight-bearing becomes more effective, the entire system benefits.

 

That includes bones, joints, balance, and overall vitality.

What Research Says About Bones for Life

Current Evidence and Ongoing Research

There is not a body of Bones for Life osteoporosis research yet. I wish there were. But that is not unusual. We still do not have a strong body of evidence showing that standard exercise programs consistently reverse osteoporosis in older adults either.

 

What we do have is a 2017 pilot study that Carol Montgomery, Drs. Shereen and Mark Farber, and I produced looking at effects on balance, vitality, and mental health in an aging population. The results were encouraging.

 

For me, the method passes what one of my late colleagues, Nancy Magnuson, a Feldenkrais practitioner, called “the sniff test” for bone health. Physical therapists and rehab professionals who come through the work do not look at it and say, “This makes no sense.” Instead, they recognize that it addresses important factors associated with healthy aging:

  • Balance
  • Walking mechanics
  • Fall recovery
  • Movement efficiency
  • Weight-bearing organization
  • Functional mobility
  • Decreased pain and increased comfort
  • Reduction in fear of movement and in fear of falling

Whether future research grows substantially or not, these remain important qualities for maintaining independence and reducing fall risk as we age.

Bones for Life exercises like this one use gentle, rhythmic movement to send bone-strengthening signals through the skeleton without high-impact strain. What matters about this lesson is not force. It is not intensity. And it is not trying harder.

 

Instead, it uses gentle, low-intensity vibration similar to the force created during heel strike while walking. 

 

Ruthy Alon was interested in whether that kind of rhythmic input could call the bones toward health without creating unnecessary strain.

 

The vocal rhythm matters too. Saying “Pum, pum” helps maintain a lighter effort, avoid breath-holding, and create a more organized movement pattern. It also gives the nervous system a rhythm to organize around. For some people, that alone is a big deal.

 

Safety Considerations 

 

If you have heel spurs, plantar fasciitis, a Morton’s neuroma, or balance concerns, listen to your body and modify or skip the lesson as needed.

For me, the long view is simple. I want every step to become better medicine. I want weight-bearing to be cleaner. I want the body to be more available, not more braced. When that changes, vitality changes too.

 

And it is never too late to start investing in your own health, your movement and your  life.
